Community Living Counselor - Young Adult Services (Transitional Living House) Description:

Are you passionate about supporting young adults in achieving their personal goals and living independently? Join our Young Adult Services (YAS) team in Willimantic, where you’ll provide residential support, individual counseling, and group facilitation in a unique transitional living environment. You’ll work closely with clients ages 18-25, who have been diagnosed with mental health challenges, helping them develop life skills, and participate in community activities. This role offers a rewarding opportunity to make a meaningful impact while collaborating with families, therapists, and other professionals.

YAS is a therapeutic program that provides intensive, wrap around services, including clinical, medication management, nursing, case management, vocational, recreational, transitional living and housing services.

As a Community Living Counselor, you’ll engage with clients daily, providing guidance, monitoring medications, teaching daily living skills, and supporting vocational and recreational activities. In this position, you’ll assess client needs, develop and monitor Treatment Plan Goals and Objectives, and advocate for continuity of care. You’ll also maintain accurate documentation, contribute to program planning, and participate in ongoing professional development to enhance your counseling skills. Schedule:

1st Shift:

  • Part-time (1st Shift): Friday - Monday 7:00 am - 3:30 pm (32 Hours)

2nd shift:

  • Full-time (1st Shift): Sunday - Thursday 12:30 pm - 9:00 pm (40 hours per week)
  • Part-time (2nd Shift) Friday & Saturday 12:30 pm - 9:00 pm (16 hours per week)

Requirements:

Bachelor’s Degree in a relevant human services field, or an Associate's Degree plus two years of appropriate experience required.

Skills: Knowledge of community resources, risk assessment, conflict mediation, life skills coaching, and electronic health record documentation.

Salary/Hourly Range:
$19.00 – $21.50 per hour (depending on experience)
Bilingual (Spanish/English) rate of pay: $19.96 – $22.59 per hour (depending on experience)
