THE POSITION

DESCRIPTION OF WORK

 

 

 

In this position, you will coordinate the transition of youth at the facility back into their communities. This will be done through leading an active process of discharge planning, establishing an aftercare network of community-based resources and conducting follow-up after youth leave. Your work will involve participating in the multidisciplinary process and assuming responsibility for the aftercare and discharge portions. You will plan transitions with the residents and their families while identifying the needs of the youth, their family, and social support system. Some of your duties will include administering exit interviews, attending court hearings when necessary, and coordinating with outside agencies. Additionally, you will participate in meetings, conferences, and training on a variety of topics to develop and maintain position skills.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 4 days, 9:00 AM to 5:00 PM and 1 day, 12:00 PM to 8:00 PM, Monday - Friday, with a 30-minute lunch. This may change based on operational needs.
  • Travel and overtime as needed
  • You must be willing to accept employment at North Central Secure Treatment Unit and commute to this location in Danville PA.
  • Free parking!
  • Telework: You will not have the option to telework in this position.
  • Salary: Selected candidates who are new to employment with the Commonwealth of Pennsylvania will begin employment at the starting annual salary of $59,345.00 (before taxes).

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

 

 

 

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) (must have been employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment); or
  • Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration) and a bachelor’s degree in social work or social welfare; or
  • Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare; or
  • A master's degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree.

Legal Requirements: 

  • A conditional offer of employment will require a medical examination and drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
