Physical Therapist - Home Health - West St Paul | Front-Loaded PTO + Bonuses

Schedule: Full Time – 80 hours/pay period – Monday to Friday – 8 a.m. to 4:30 p.m.

As a Physical Therapist, you’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well-being.

At Vivie, we value our people and offer a competitive pay range of $45.00/hr to $55.00/hr (hourly non-exempt) based on qualifications, experience, and location. Our comprehensive benefits package includes health coverage, continuing education support, and wellness programs.

As a Physical Therapist, you will: 

  • Deliver Direct Client Care – Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
  • Maintain Accurate Client Records – Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
  • Facilitate Follow-Up Programs – Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes. 
  • Collaborate and Communicate Effectively – Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
  • Ensure Regulatory Compliance – Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of nature and level of work assigned and are not necessarily all inclusive. 

 This job also requires: 

  • Current Physical Therapist license in the state of Minnesota.
  • Strong collaboration and teamwork skills.
  • Clear and compassionate communication abilities.
  • Proficient in English for effective communication with staff, clients, and visitors. 
  • Successful completion of a state-mandated background check.
  • Able to physically perform the duties required by the position.
  • Must obtain CPR certification within 6 months of hire.
