Southside Community Health Services (SCHS) is a mission-driven nonprofit clinic in South Minneapolis seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care in a collaborative community health setting. This role supports SCHS’s commitment to accessible, equitable, whole-person care.
About the Role
The Advanced Practice Provider provides comprehensive primary care services for diverse and underserved populations, including preventive care, chronic disease management, acute care, and patient education. This position emphasizes team-based care, clinical excellence, and a strong community health focus.
Key Responsibilities
- Provide comprehensive outpatient care, including preventive, acute, and chronic disease management
- Maintain full patient contact hours based on FTE and participate in rotating on-call coverage (as applicable)
- Collaborate with interdisciplinary teams to coordinate holistic patient care
- Complete timely documentation, chart closure, peer reviews, and EHR best practices
- Ensure compliance with SCHS clinical protocols, quality standards, and credentialing requirements
- Supervise and precept students as assigned
- Complete administrative tasks, forms, and required trainings
- Participate in staff meetings, committees, and clinic operations

Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Board certified with active Minnesota licensure
- Current DEA registration and BLS certification
- Minimum 1 year of experience in a clinic-based or community health setting (2+ years preferred)
- Experience with Epic preferred
- Bilingual (English/Spanish or Somali) preferred
