Blended Case Manager

Exciting Opportunity for Current College Students to Work FULL TIME while completing their education!!!  

Join our Blended Case Management (BCM) Team as a Blended Case Manager where you will assist children and adults facing mental health challenges to obtain needed mental health, physical health, social, financial, vocational, educational, and other necessary supports. Gain valuable experience toward your career in the mental health field. Our Director is a leader in trauma-informed care training and will provide valuable mentorship.  This is a MOBILE position requiring regular travel throughout Bucks County.

What You Offer: 

  • Minimum Education: HS Diploma with at least 12 college credits in a related social services field
  • At least 1-year direct client care experience in a behavioral health setting
    • Internship and volunteer experience will be considered.
  • Background in Case Management related experience preferred.
  • Experience in providing community-based (mobile services) preferred.
  • Proficiency with completing collaborative documentation in an Electronic Health Record (EHR) preferred.
  • Valid Driver’s License
  • Excellent verbal and written communication skills
  • Computer proficiency
  • Ability to perform the physical requirement of the job description, with or without accommodation

What You’ll Do: 

  • Provide community-based Blended Case Management services focused in Central Bucks County
  • Meet with individuals and family members in their homes as well as their community
  • Establish and maintain effective working rapport with individuals, associates, and agency personnel
  • Link individuals and families to needed services, resources, and programs to positively impact an individual’s overall well-being
  • Collaborate with other members of the LVF treatment team, as well as other providers and organizations
  • Complete recovery-oriented and thorough documentation within LVF’s electronic health record
