Position Summary
RTV is seeking dedicated and mission-aligned Case Managers to serve as contractors supporting youth and families. In this role, you will coordinate individualized care, conduct home and in the office sessions. This position plays a critical role in executing RTV’s holistic service model bringing together social-emotional wellness, academic intervention, and family engagement.
Case Managers will work independently and collaboratively under the guidance of RTV’s Case Management Supervisor, while contributing to the delivery of data-driven, trauma-informed support services that reflect RTV’s commitment to measurable impact.
Key Responsibilities
Client Support & Coordination
Develop individualized service plans based on client assessments and identified needs.
Engage individuals and families through in-person meetings, phone calls, and virtual support.
Conduct regular visits and follow-ups to ensure progress and care continuity.
Connect individuals and families with relevant community, educational, and healthcare services.
Track attendance, behavioral outcomes, and progress toward goals.
Serve as a liaison between schools, families, and partner organizations.
Documentation & Program Evaluation
Maintain accurate and confidential electronic and paper client records.
Submit weekly service logs and outcome tracking using RTV’s data reporting system.
Participate in quality assurance checks and periodic data review meetings.
Ensure case files and documentation are audit-ready and aligned with RTV and Florida compliance standards.
Representation & Administration
Attend and contribute to RTV team meetings, trainings, and school-based coordination calls.
Represent RTV at community events, stakeholder meetings, and provider conferences as needed.
Support administrative functions including document organization, data entry, and reporting.
Qualifications
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
Minimum of 2 years’ experience in clinical case management or youth and family services.
Strong understanding of wraparound service models and trauma-informed care.
Ability to work independently while meeting strict timelines and deliverables.
Excellent documentation, organizational, and interpersonal skills.
Proficient in Microsoft Office and case tracking/data platforms.
Must have reliable transportation for home visits and school-site travel.
Ability to pass a background check and complete RTV’s contractor onboarding.
Contract Details
Type: Contractual (1099), no benefits included
Compensation: $40,000–$45,000 annually, based on experience, hours committed, and qualifications
Schedule: Monday–Friday, generally 9:00 AM–5:00 PM with flexibility for after hours for home visits or client needs
