Acute Rehab Nurse

• Department: Acute Rehab
• Schedule: Night shift, 36hrs/wk, 7pm to 7:30am,  with every other week-end and holiday rotation.
• Hospital: Sacred Heart Rehab
• Location: Mequon, WI

Acute Rehab Nurse Responsibilities:

Team Player: Coordinates with all care team members (doctors, specialists, therapists, social workers) to ensure unified patient care and optimal outcomes.

Care Plan Pilot: Executes, monitors, and adjusts the patient's care plan using critical thinking and informed, quick decisions based on observation and team input.

Patient Care Administrator: Accurately administers medications, conducts tests, and performs treatments/procedures as ordered by the physician.

Patient Advocate: Continuously monitors the patient's status, meticulously records all findings and actions in the EHR, and promptly reports significant changes to the doctor and team.

Discharge Planner and Educator : Initiates discharge planning upon admission, collaborating with the patient, family, social work, case management, and physicians to arrange post-care services and provide essential education for a safe transition home.
