Case Manager

Part of the person centered planning team to assist in the coordination and provision of service.  Positions available working with adults who experience a serious and persistent mental illness.  Responsible for assessment, plan development, linking, coordinating, advocacy, etc. 

Qualifications 

Minimum of a bachelor’s degree in social work, psychology, or related human services degree. Current Michigan social work licensure along with experience providing service to adults who have a serious and persistent mental illness preferred. Must have a valid, unrestricted driver's license, proof of insurance and availability of reliable transportation for outreach work.
