Social Worker

Polk County Social Services is seeking an adult services social worker!

This position works in the Adult Behavioral Health Unit, providing case management to adults with mental illness and substance use disorders. Duties include service coordination, civil commitment court involvement, MnCHOICE assessments for disability waiver participants, and adult protection responsibilities such as responding to maltreatment reports, assessing safety, and coordinating protective services.
