Now Hiring: Community Interventionist

Location: Lehigh and Northampton Counties

Type: Full-Time

Pay Rate: 19.00 per hour

 

Purpose:

To provide quality community interventions for adolescents and their families in the community.  Primary responsibilities are to act as a liaison and maintain communication between The Children's Home of Reading, referral/other agencies; represent the agency at all court hearings and interagency meetings; identify & utilize community services/supports as needed, act as advocate, act as mentor, discharge planning and transport clients.

Essential Functions and Duties:    

DEMONSTRATES ABILITY IN THE FOLLOWING CORE AND PROGRAM COMPETENCIES:

  1. Excellent verbal/written communication skills.
  2. Excellent time management skills.
  3. A strong familiarity with various community resources.
  4. Facilitates/co-facilitates groups for adolescents.
  5. Completes all required documentation including service plans/reviews, weekly summaries and discharge summaries.
  6. Therapeutic crisis prevention and intervention; First Aid and CPR; infectious disease control policies and procedures; emergency fire procedures/evacuation; Emergency medical procedures; Child Protective Services Law; Child abuse recognition and staff responsibility; DPW regulations section 3810; Vehicle safety.
  7. Ability to assess, interpret and effectively document data specifically related to the care of adolescents.
  8. Assist families with obtaining/maintaining basic entitlements and benefits as necessary.
  9. Telephone system and proper telephone etiquette.
  10. Use of general office machines: copy machine, fax machine.
  11. Unit/program policies and procedures.
  12. Agency policies and procedures.
  13. Continuous Quality Improvement (CQI): theory and processes.

Requirements:

  1. Must be at least 21 years of age.
  2. Valid PA driver’s license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage.
  3. Bachelor’s degree in social work, psychology or other related field preferred.
  4. One to three years experience preferred working in a human services field (CASSP system-related setting preferred).
  5. At least three (3) written references that demonstrate past educational and/or employment experience.
  6. Successful completion of core development orientation program.
  7. PA Act 33/34/73 clearances and proof of exclusions
  8. Must meet all requirements of Title 8, United States Code, Section 1324-A of the United States Department of Justice/Immigration and Naturalization Services Employment Eligibility Verification.  
  9. Pre-employment physical from a licensed physician (after conditional job offer) verifying person has no communicable diseases or physical conditions that might endanger the health and safety of children or prevent the person from performing essential job responsibilities.
