Speech-Language Pathologist - Skilled Nursing Facility - San Jose, CA

We are seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team in a Skilled Nursing Facility (SNF) / Post-Acute Care setting in San Jose, CA. The SLP will assess, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders. This role plays a key part in improving residents’ safety, quality of life, and functional independence in a medically complex setting.

 

Key Responsibilities

 

Clinical Evaluation & Treatment

  • Conduct comprehensive evaluations of speech, language, cognitive-communication, and swallowing functions.
  • Develop and implement individualized treatment plans for residents recovering from illness, injury, or surgery.
  • Provide evidence-based therapy, including dysphagia management, communication therapy, voice treatment, and cognitive rehab.
  • Perform Modified Barium Swallow (MBS) or Fiberoptic Endoscopic Evaluation of Swallowing (FEES) if credentialed (optional).
  • Recommend appropriate diet textures, compensatory strategies, and therapeutic interventions for safe swallowing.

Documentation & Regulatory Compliance

  • Complete timely and accurate documentation in compliance with SNF, Medicare, and state regulatory requirements.
  • Prepare evaluations, progress notes, care plans, and discharge summaries.
  • Track and document minutes for PDPM/Medicare guidelines.
  • Maintain confidentiality and adhere to professional and facility-specific standards.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current California SLP License (or eligibility to obtain).
  • Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates may be considered depending on facility.
  • Experience in SNF, post-acute care, or medical settings preferred.
  • Strong clinical reasoning, communication, and documentation skills.
