General Description:

 

Under the direction of the Administrative Director and/or Clinical Director, the Care Coordinator serves as a liaison between children/youth, families, the school team, and other IBHS team members to make sure the individual treatment plan put in place by the care team is working effectively for all involved. The Care Coordinator will assess youth and family social determinants of health and provide supports to address identified physical and behavioral needs. The Care Coordinator is to assure that the consumers receive services identified on the psychiatric and psychological evaluation throughout the duration of treatment. The Care Coordinator assures that the consumer's needs are coordinated among other social service programs, agency departments, outside health care agencies, and funding sources.  


Duties and Responsibilities


School/Family Partnership

  • Demonstrate the ability to establish positive relationships with school officials, youth and families, and all members of the child serving system
  • Coordinate care with the school (including related therapies such as speech, occupational, and physical therapies), other providers, and service systems at the time of admission to IBHS and throughout the course of care to discharge
  • Work in conjunction with the school and family to facilitate successful transition into and back from out-of-school placements such as acute inpatient, partial hospitalization, and residential treatment facilities 
  • Monitoring school census and staffing for assigned schools
  • Ensure the day-to-day operations developed by their supervisor are executed seamlessly
  • Coordination with the treatment team, child/youth, family members, and other involved agencies as necessary. This may include visits to the family home, school, or other community-based locations
  • Document all services provided within 48 hours of service delivery in the electronic health record (EHR). This includes care coordination notes for outreach phone calls, emails and any other correspondence related to a client’s services.


Team Success

  • Demonstrate the ability to communicate effectively and efficiently with all internal and external stakeholders
  • Complete consent for treatment and authorization to release information with child/youth, and/or guardians as part of the intake process and renew such documents annually or as needed
  • Ensure all members of the IBHS team are aware of case status, including, level of engagement, social determinant of health needs
  • Initiate scheduling all new referrals for a level of care assessment or evaluation and notify all relevant parties of requested services and staffing type (i.e medication management, behavioral health technician)
  • Ensure follow up on non-clinical recommendations from the Level of Care Assessment, psychological and/or psychiatric evaluations as well as Community Behavioral Health (CBH) recommendations, and the direction of the treatment team, and supervisor
  • Review and update master case list to reflect authorization disposition, services provided and distribute this information to appropriate team members
  • Participate in all team meetings, staff meetings, school meetings, and any additional departmental meetings
  • Active participation and completion of all mandatory trainings set forth by all stakeholders including CBH within the timeframes required
  • Provide coverage on other teams as directed by Program leadership
  • Cooperate and communicate as a multi-disciplined staff team member through formal meetings, consultation, and committee participation
  • Models traits of a responsible team member, executing job responsibilities, open communication, good follow through; supports team members to do the same
  • Responds promptly to the concerns, needs, interests of children/youth, parents, and guardians involved in the IBHS program
  • Perform other related duties as assigned

Direct Member Services

  • Engage children/youth, and parents or guardians in universal screening for bullying, trauma, externalizing/internalizing behaviors, and suicide and related risks 
  • Complete social determinants of health assessment on families participating in IBHS
  • Demonstrate commitment to being trauma-informed and maintaining a therapeutic environment and community that promotes respect, safety and accountability as the basis for all interactions with clients and all other stakeholders
  • Demonstrates knowledge and utilization of trauma-informed practices, including understanding of impact of trauma on development, strengths based and person-centered practice, resilience focus and commitment to growth and change


 

Program Monitoring & Compliance

  • Ensure compliance with all IFSI’s policies and procedures, including confidentiality and acting as a mandated reporter in accordance with the law, policies and procedures
  • Implement all documentation requirements: Reports are complete, thorough, timely, and consistent with Data, Assessment, Plan (DAP) format
  • Other duties as assigned

Qualifications:

Requires one of the following:


Bachelor's Degree (or higher) in a Human Service related field (Psychology, Social Work, Sociology, Education, Criminal Justice, Human Services, Behavioral health, or other social services related field)

or

  • High School Diploma or equivalency and experience working with children and adolescents
  • Valid driver’s license
  • Computer proficiency

And

Have a minimum of 5 years of experience working with children and adolescents.
