Located at our new state-of-the-art Windham Regional Health and Wellness Center at 140 North Frontage Road, Mansfield Center, CT.

Case Manager - Young Adult Services (YAS) Description:
Are you ready to make a meaningful difference in the lives of young adults with mental health challenges? As a Case Manager with our Young Adult Services Program in Willimantic, you’ll build connections with individuals aged 18-25, helping them achieve safe housing, access to quality healthcare, meaningful relationships, and community involvement. This role allows you to provide individual support, group counseling, and transitional living guidance while coordinating services that empower clients to reach their personal goals and live more independently.

In this position, you’ll assess client needs, develop and monitor Treatment Plan Goals and Objectives, and advocate for continuity of care. You’ll provide daily support, including counseling, education, transportation, and skill-building to enhance independent functioning. Strong interpersonal and organizational skills are essential as you collaborate with clients, families, and professionals, manage multiple tasks, and maintain accurate documentation. YAS is a therapeutic program that provides intensive, wrap around services, including clinical, medication management, nursing, case management, vocational, recreational, transitional living and housing services.

Schedule: Monday through Thursday, 9:00 a.m. – 5:30 p.m., Friday, 8:30 a.m. – 5:00 p.m.

Qualifications:
Bachelor’s Degree in a relevant human services field or Associate’s Degree in a relevant human services field plus two years of relevant experience required.

Skills: Knowledge of community resources, risk assessment, conflict mediation, life skills coaching, electronic health record documentation

Salary/Hourly Range:

  • $19.00 - $21.50 per hour (depending on experience)
  • Bilingual (Spanish/English) rate of pay $19.96 - $22.59 per hour (depending on experience)
