LivingWell Institute provides holistic lifestyle education and family restoration services for individuals and families navigating trauma, chronic pain, addiction recovery, anxiety, and depression. Our programs take place in the community — in clients’ homes, at our Hershey and Harrisburg hubs, and at partner locations throughout the region.
We believe change happens through relationships, practical support, and whole-person care.
Position Overview
As a Family Mentor / Care Provider, you will support individuals and families through consistent, relationship-based care. LivingWell provides professional training in our Integrative Health Coaching model.
This role combines hands-on support, mentorship, and community engagement.
Key Responsibilities
- Provide holistic, practical support to individuals and families, including: Health and wellness activities, Cooking and nutrition support, Parenting support, Budgeting, education, and resource navigation.
- Provide transportation to LivingWell services, appointments, and community resources
- Assist with group education and activities at the Hershey office, Harrisburg hub, RiverExchange, and other locations
- Collaborate with a supportive team to ensure continuity of care
- Other related duties as needed

Requirements
- Ability to obtain state and federal background clearances
- Narcan training (provided if needed)
- Reliable transportation
- Availability for Morning hours (typically 8:00 AM–12:00 PM) some evenings and weekends, based on client needs
- Ability to complete timely and accurate documentation of interactions with individuals and families to track progress and support continuity of care.
