What You’ll Do
- Deliver skilled therapy services under a PT plan of care
- Document patient visits accurately and in compliance with Medicare standards
- Communicate patient progress and concerns with supervising PT and care team
- Educate patients and caregivers on exercises, safety, and functional independence
Who We’re Looking For
- Licensed or license-eligible Physical Therapist Assistant (PTA) in Connecticut
- Strong interpersonal and communication skills
- Patient-centered mindset with a passion for home health care
- Reliable transportation to visit patients in their assigned territories
