TCM Care Manager Supervisor
Role Overview
We’re seeking a TCM Supervisor to provide leadership, oversight, and support to our Tailored Care Management team. This role ensures high-quality, compliant service delivery while coaching Care Managers, improving outcomes, and maintaining fidelity with NC Medicaid and Tailored Plan standards.

What You’ll Do
- Lead and supervise TCM Care Managers, including training, coaching, performance evaluation, and case consultation.
- Ensure compliance with the 6 Core TCM Functions and NC Medicaid/Tailored Plan requirements.
- Conduct documentation reviews, quality audits, and support corrective action plans.
- Oversee caseload distribution, acuity levels, and team productivity.
- Support Care Managers with complex cases, crises, and transitions of care.
- Partner with leadership to improve workflows, quality measures, and client outcomes.
- Use data to identify trends and drive continuous improvement.
- Build strong relationships with members, families, and community partners.
What We’re Looking For
- Bachelor’s degree required; Master’s degree preferred (Social Work, Human Services, Nursing, or related field).
- 3–5 years of experience in behavioral health, care coordination, or case management.
- Supervisory or team leadership experience preferred.
- Knowledge of Tailored Care Management, integrated care models, and NC Medicaid.
- Strong communication, problem-solving, documentation, and organizational skills.
- Ability to mentor staff and lead with empathy, accountability, and professionalism.
- LCSW, LCMHC, RN, or related license/certification preferred (not always required).
- Valid driver’s license and reliable transportation, when needed.
