CME Care Coordinator
The OhioRISE Care Coordinator is responsible for providing high-quality, family-centered care coordination services to youth with complex behavioral health and multisystem needs enrolled in the OhioRISE (Resilience through Integrated Systems and Excellence) program. The Care Coordinator works collaboratively with the child/youth, family, and system partners to develop, implement, and monitor an individualized Child and Family-Centered Care Plan that promotes stability, resilience, and wellness.
Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
Required Qualifications: High School Diploma plus 3 years’ experience in at least one of the above.
Associates or bachelor’s degree plus 2 years’ experience in at least one of the above.
Master’s degree plus 1 year experience in at least one of the above.
