Adult Case Manager (3059)

RESPONSIBILITIES
Provides assessment, linkage, and advocacy services to individuals with developmental disability; develops and implements individual service plans; completes required documentation; and complies with all regulations applicable to program services.

REQUIREMENTS
Minimum Requirements
M1: Bachelors Degree in a Human Services field (degree subject to approval)
OR
Registered Nurse with a valid Registered Nurse license to practice in Virginia
M2: Valid drivers license with a safe driving record

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.

ANNUAL SALARY RANGE
$43,722 - $76,514
5% Increase for Qualified Mental Health Professional
