Substance Abuse Case Manager
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Clinical Case Management is a support service that involves the process of linking and coordinating components of a service system to ensure the client's treatment/rehabilitation needs are met. Case management facilitates client-service system through interaction and movement towards planned changes through development of appropriate casework plans that include service provision and client advocacy.
SPECIFIC DUTIES AND RESPONSIBILITIES:
- Provide a minimum of 25 hours of direct service per week for substance abuse case management.
- Ensure quality patient care per JCAHO, Bureau of Primary Health Care, and RHCC standards.
- Coordinate and oversee the following activities: assessment, development, and ongoing revisions to PCP; monitoring/implementation of PCP.
- Coordinate and oversee initial and ongoing activities.
- Perform other necessary duties as required by Robeson Health Care Corporation to meet the goal of providing primary health care services.
- Convene treatment/consultation team for staffing purposes. The case manager frequently convenes teams for the purposes of (a) collaborating during crisis when an applicant needs to be admitted on an emergency basis, (b) planning for services, and (c) coordinating admission/re-admission with a therapist.
- Educate, consult and negotiate with other human service agencies, both public and private, concerning admission and treatment.
- Provide comprehensive assessment of the client's rehabilitation needs and determine, in conjunction with the counselor, an appropriate treatment plan.
- Implement and monitor the treatment plan including evaluation outcomes.
- Offer support services that involve the process of linking and coordinating components of the service system to insure that the client's treatment/rehabilitation needs are met.
- Plan for need/problem resolution through the identification of an appropriate service network inclusive of all available resources.
- Function as an advocate for the client.
- Conduct discharge planning and aftercare follow-up.
- Maintain medical records according to Medicaid billing standards.
- Responsible for conducting both SACOT/SAIOP groups as necessary.
- Execute other duties as assigned.
QUALIFICATIONS:
- Bachelor’s or advanced degree in a human service related field with documentation of at least two years of supervised experience in the profession of alcoholism and drug abuse counseling or related field.
- Understanding of substance abuse and chemical dependency as defined in the DSM-IV.
- Understanding of addictions and knowledge of the impact of addiction on women and families. Knowledge of 12-step programs.
- Knowledge of perinatal programs aimed at rural women and minorities.
- Experience in the techniques of casework, group work, or community organizations.
- Familiarity with the community and its resources. Individual must be familiar with other agencies and with what they offer.
- Ability to correlate needs with possible solutions; ability to synthesize information effectively in order to communicate successfully both orally and in writing.
