Home Health Clinical Case Manager (RN)

 

Job Type: Full-Time

Location: Fort Wayne, Indiana

Reports To: Director of Nursing

 

Position Overview:

We are seeking a compassionate, skilled, and self-motivated Registered Nurse (RN) to join our Home Health team as a Clinical Case Manager. In this role, you will provide direct nursing care to patients in their homes, following an individualized plan of care under the direction of a physician. The Home Health RN is responsible for assessing patient needs, developing care plans, and delivering high-quality, patient-centered care while promoting recovery, independence, and overall health.

Key Responsibilities:

  • Conduct initial and ongoing assessments of patient health status and needs.
  • Develop, implement, and revise individualized care plans in collaboration with physicians and other healthcare team members.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient responses to treatment and interventions.
  • Educate patients and family members on disease processes, self-care techniques, and health maintenance.
  • Coordinate care with other health professionals, including physical therapists, social workers, and home health aides.
  • Ensure compliance with all clinical and regulatory standards including HIPAA, OSHA, and CMS guidelines.
  • Maintain accurate, timely, and complete documentation in the Electronic Medical Record (EMR) system.
  • Participate in on-call rotations, team meetings, and case conferences as required.

 

Qualifications:

  • Current RN license in the state of practice (required).
  • Minimum of 1 year of nursing experience, preferably in home health, hospice, or acute care.
  • BLS Certification (Basic Life Support) – current and valid.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Excellent clinical, communication, and time-management skills.
  • Ability to work independently and manage multiple patient visits per day.
  • Proficiency with electronic charting systems (EMR).
  • Experience with OASIS documentation and Medicare-certified home health agencies.

 

Physical Demands:

  • Ability to travel to patient homes and work in various home environments.
  • May require lifting, transferring, and positioning patients.
  • Frequent standing, walking, bending, and reaching.
