Home Health Clinical Case Manager (RN)
Job Type: Full-Time
Location: Fort Wayne, Indiana
Reports To: Director of Nursing
Position Overview:
We are seeking a compassionate, skilled, and self-motivated Registered Nurse (RN) to join our Home Health team as a Clinical Case Manager. In this role, you will provide direct nursing care to patients in their homes, following an individualized plan of care under the direction of a physician. The Home Health RN is responsible for assessing patient needs, developing care plans, and delivering high-quality, patient-centered care while promoting recovery, independence, and overall health.
Key Responsibilities:
- Conduct initial and ongoing assessments of patient health status and needs.
- Develop, implement, and revise individualized care plans in collaboration with physicians and other healthcare team members.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient responses to treatment and interventions.
- Educate patients and family members on disease processes, self-care techniques, and health maintenance.
- Coordinate care with other health professionals, including physical therapists, social workers, and home health aides.
- Ensure compliance with all clinical and regulatory standards including HIPAA, OSHA, and CMS guidelines.
- Maintain accurate, timely, and complete documentation in the Electronic Medical Record (EMR) system.
- Participate in on-call rotations, team meetings, and case conferences as required.
Qualifications:
- Current RN license in the state of practice (required).
- Minimum of 1 year of nursing experience, preferably in home health, hospice, or acute care.
- BLS Certification (Basic Life Support) – current and valid.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Excellent clinical, communication, and time-management skills.
- Ability to work independently and manage multiple patient visits per day.
- Proficiency with electronic charting systems (EMR).
- Experience with OASIS documentation and Medicare-certified home health agencies.
Physical Demands:
- Ability to travel to patient homes and work in various home environments.
- May require lifting, transferring, and positioning patients.
- Frequent standing, walking, bending, and reaching.
