Mental Health care coordinator
Indeed Post: Registered Nurse
Position Responsibilities:
- Provide mental illness and health education to clients, client’s families, staff, and community.
- Assist in refilling medications with psychiatrist order as needed and provide liaison to pharmacy providers. In addition, they will need to speak with laboratory providers regarding reviewing lab reports.
- Receive, review, and respond as appropriate to calls regarding physician ordered medication or medication issues.
- Under direction, arranges for hospital admission of clients, and facilitates the referral of patients to appropriate health facilities, agencies, and resources.
- Participate in treatment planning, program meeting, in-service and staff training as well as individual and group supervision. Provide consultation on medical issues.
- Complete in a timely manner documentation and billing information in accordance with the guidelines developed for Medicaid services and by the New Jersey Division of Mental Health Services.
- Performs other tasks as deemed necessary by supervisor.
Requirements:
- BS/BSN from an accredited education institution
- Valid RN License from the State of New Jersey
- Valid and current New Jersey Drivers License and be 21 years or older
