Klamath Health Partnership is seeking a compassionate, skilled Physical Therapist to join our outpatient rehabilitation team. What You’ll Do
- Conduct comprehensive evaluations and develop individualized treatment plans in partnership with patients.
- Deliver evidence-based physical therapy interventions using therapeutic equipment, manual therapy, and assistive devices.
- Supervise Physical Therapist Assistants, Aides, and students as appropriate.
- Coordinate care with referring providers and interdisciplinary teams to support holistic health goals.
- Document care accurately and timely using EClinicalWorks EMR.
- Educate patients and families on home exercise programs, mobility strategies, and device use.
- Ensure compliance with CMS standards, including MIPS reporting.
- Maintain confidentiality and uphold professional ethics as outlined by the Oregon Board of Physical Therapy.
Qualifications
- Degree in Physical Therapy (Doctorate, Master’s, or Bachelor’s) from a CAPTE-accredited program.
- Oregon PT license (or eligibility to obtain).
- BLS certification (required within 30 days of hire).
- DHS background check required for designated roles (PL540 & PL580).
- Experience in FQHC settings preferred.
- Strong communication and teamwork skills.
- Commitment to culturally responsive care and confidentiality.
- Availability for occasional evenings and weekends.
