Summary: Under the supervision of an RN, and within the scope of practice for a Licensed Practical Nurse:
- Provides health education according to the teaching plan established by the RN.
- Provides routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
- Observes signs and symptoms, and reports patient’s reactions to treatments; including drugs and changes in the patient’s physical or emotional condition, to the physician and RN.
- Documents:
- any services rendered to the patient,
- changes noted in the patient’s condition,
- changes noted in family and/or home situations.
- Documents communication with RN Case Manager and physician.
Minimum Qualifications:
- NYS Licensed Practical Nurse credentials/unencumbered.
- Knowledge of State and Federal regulations, as well as payer guidelines.
- NYS Driver’s License·
- CPR Certification.
- Demonstrate critical thinking.
- Strong organizational and time management skills, possesses ability to multi-task.
- Strong EMR documentation skills.
- Clinical competency within the scope of practice of an LPN.
- Demonstrated effective communication and collaboration with multidisciplinary care team.
