Summary:  Under the supervision of an RN, and within the scope of practice for a Licensed Practical Nurse:

• Provides health education according to the teaching plan established by the RN.
• Provides routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
• Observes signs and symptoms, and reports patient’s reactions to treatments; including drugs and changes in the patient’s physical or emotional condition, to the physician and RN. 
• Documents:
  • any services rendered to the patient, 
  • changes noted in the patient’s condition, 
  • changes noted in family and/or home situations.
• Documents communication with RN Case Manager and physician.

Minimum Qualifications:

• NYS Licensed Practical Nurse credentials/unencumbered.
• Knowledge of State and Federal regulations, as well as payer guidelines.
• NYS Driver’s License·
• CPR Certification.
• Demonstrate critical thinking.
• Strong organizational and time management skills, possesses ability to multi-task.
• Strong EMR documentation skills.
• Clinical competency within the scope of practice of an LPN.
• Demonstrated effective communication and collaboration with multidisciplinary care team.
