What You’ll Do:

  • Participate in continuing education/professional development activities.
  • Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others.
  • Adhere to policies and practices of applicable professional organizations, and Progressive Care Therapy, Inc.
  • Adheres to applicable dress code.
  • Adjusts personal schedule to meet needs of facility when requested.
  • Participate in clinical education activities for students/volunteers upon request.
  • Attends/presents at facility in-service sessions as scheduled.
  • Attends external professional growth activities as scheduled and communicated information to the team.
  • Protects patient confidentiality at all times.
  • Communicates professionally as evidenced by client satisfaction.
  • Is familiar with and acts according to content applicable of the professional code and company handbook.
  • Contribute to client relations and marketing activities.
  • Attends and participates in community educational/promotional activities as requested.
  • Makes positive contribution and promotion of Progressive Care Therapy.
  • Maintain licenses/certifications and memberships in applicable professional organizations.
  • Provide age and diagnosis appropriate therapeutic interventions as directed by the physical therapist.
  • Evaluate patient response to treatment and provide feedback to the rest of the team.
  • Document the course of patient care including progress made and continuing need for treatment.
  • Provide patient and family education on tasks and resources that will assist with continued functional independence.
  • Therapeutic interventions result in positive outcomes as judged by patient satisfaction.
  • Uses positive feedback to promote patient
  • participation in their rehabilitation program.
  • Other members of the team are consulted to coordinate therapeutic activities.
  • Subjective information is relevant in documenting patient response to treatment and functional changes noted
  • Patient response to the care plan is evaluated as evidenced by patient achievement of goals and/or goal revision, the care plan is updated as needed.
  • Information is also provided to patient/family/caregivers regarding current status, program planning and potential discharge plans.
  • Interventions and effects are documented every session according to policy and procedure.
  • Progress notes are completed with objective data and goal progression.
  • Input is provided to the physical therapist regarding the discharge summary.
  • Effective and appropriate Home Exercise Programs are completed and reviewed with each client to support independence prior to discharge.
  • Effectiveness of education measured by family/client verbal feedback and patient satisfaction surveys.
  • Coordinate treatment with other disciplines.
  • Participate in rehab team meetings.
  • Assist the physical therapist in providing team members with in-service/cross training on physical therapy knowledge and skill areas as needed.
  • Contribute to development of improved efficiency and productivity of facility functions.
  • Participates in interdisciplinary care conferences and provides/seeks input to/from others on patient care.
  • Attends rehab meetings and offers ideas/solutions in problem solving situations/activities regarding the program.
  • Attends in-service sessions.
  • Assists with leading in-service sessions as requested/volunteered.
  • Assists in orienting other staff to the physical therapy functions of the facility.
  • Offers positive suggestions/solutions to improve service delivery and facility operations.
  • Contributes to and cooperates with the implementation of system improvements.
  • Offers to assist other staff and is flexible in carrying out shared team functions including special projects.
  • Utilizes resources appropriately and uses/maintains equipment in order to prolong working life.What We’re Looking For:
  • Graduate of an accredited Physical Therapy program.
  • Current state license and certifications.
  • Experience with adult and geriatric populations preferred.
  • Openness to learning aquatic therapy techniques.

Job Type: Full-time

Pay: $87,000.00 - $92,000.00 per year
