Substance Use Disorder Case Manager

We are seeking Substance Use Disorder Case Manager to join our team in our Delaware County, PA.

 

Earn $22.38 per hour with benefits for a 37.5 hour week, or $24.38 per hour with the selection of enhanced pay and a scaled back benefits package.

 

Position Details

The office is based out of Sharon Hill and will service clients residing in Delaware County, PA. Caseload not to exceed 30 active clients. The Substance Use Disorder Case Manager under the supervision of the Program Manager, is an individual who advocates for and connects individuals to community resources to provide opportunities and supports for individuals with mental health and co-occurring challenges. The Case Manager will work with community resources such as DPW and SSA, mental health and drug and alcohol programs, adult and juvenile probation, payers and the Office of Behavioral Health.

The ideal candidate would possess:

  • A bachelor's degree in Social Work or a related field.
  • 1 year direct care experience with the drug and alcohol / SUD population preferred.
  • Clean, valid DL and reliable transportation.
