Developmental Disabilities Case Manager
Primary Function:
- Provide leadership to the team as single point of coordination in order to complete person-centered assessment and Individual Service Plan.
- Prepare forms, reports, correspondence, case summaries and progress notes as needed.
- Complete Case Notes to be billed for Medicaid for case management services.
- Maintain and update client files as needed.
- Provides advocacy for individuals and families by making referrals, arranging evaluations and informing families of services.
- Act as team lead for meetings; coordinate all communication between providers and natural supports and individuals and families served.
- Assist with provider selection.
- Assist with eligibility determination or complete eligibility determination if applicable.
- Participate in Education and Habilitation programming as requested by family and approved by rules.
- Monitor services to assure health and safety and service satisfaction and plan compliance.
- Ensure that emergency services are provided immediately on a 24-hour basis when emergency situations develop.
- Ensure referral and coordination of services with other agencies, without duplication of services.
- Review, investigate, track and monitor unusual incident reports and report Major Unusual Incidents to include alleged abuse and/or neglect in accordance with DODD guidelines.
- Assist with any request for information from other agencies with properly documented release of information.
- Assure program compliance with all applicable laws, statutes, rules, regulations, codes standards and agency policy.
- All other duties as assigned.
Knowledge, Abilities and Skills:
- Ability to work flexible schedule and to be on-call during evenings, weekends and holidays.
- Excellent communication skills (verbal, written and interpersonal) for effective interaction with internal and external customers.
- Ability to sit up to 2 hours continuously to operate computers and other information technology.
- Ability to establish a positive work environment.
- Demonstrated problem solving, time management and organizational skills.
- Demonstrated personal computer skills required with proficiency in the Microsoft Office suite referred.
Education:
Bachelor's degree in social work, Special Education or another related field necessary to obtain/maintain the required Ohio Department of DD certification.
Certification:
- Ohio Department of DD Service and Support Administration Certification
- CPR and First Aid Certification
Licensure:
Valid State of Ohio Driver's License, with good driving record
Other:
- Must have proficient computer skills and the ability to use current and modern technology.
- Good attendance is an essential function of this position in order to provide the consistency in service provision as is needed by the clientele served.
