Job description:
GENERAL DUTIES AND RESPONSIBILITIES:
The Nurse Assistant II works as part of the clinic provider team and assist the Medical Providers and Clinic Nurse in the provision of primary health care. The Nurse Assistant II works under the supervision of the Nurse and the delegation of these activities are at the discretion on the Lead Nurse and Director of Nursing. The primary focus of the nursing staff is the delivery of quality health care in a timely and cost-efficient manner.
SPECIFIC DUTIES AND RESPONSIBLITIES:
REFERRAL COORDINATION:
1. Schedule appointments for special test and x-rays, such as mammograms and CT scans, with appropriate facilities and provides follow up under the direction of the provider/lead nurse/Director of Nursing
2. Ensures that complete documentation occurs in the EMR of the consult/Transition of Care/Referral.
3. Responsible for ensuring that all aspects of the referral process has occurred: a. Scheduling the appointment, b. Obtaining authorization as needed, c. Faxing patient records for continuity of care, d. Ensuring that the patient has been notified of the referral and documenting this in the EMR
4. Responsible for Referral Tracking: a. Ensuring that the patient kept appointment, b. Rescheduling any appointments that the patient missed and is willing to reschedule, c. Ensuring that the medical provider has been notified that patient missed appointment and documenting in the EMR, d. Responsible for ensuring that all correspondence related to referral has been received back in the medical center and ensuring that the correspondence is scanned in to the EMR. If needed, will assist with the scanning of these documents into the EMR.
5. Responsible for following all RHCC Policies and Procedures for Referral Process/Referral Tracking
CERTIFIED NURSING ASSISTANT II:
1. Ensure quality patient care per Joint Commission, Bureau of Primary Health Care, NCBON and RHCC standards.
2. Performs other duties as assigned by the Robeson Health Care Corporation to meet the goal of providing primary and preventive health care services.
3. Calibrates adult and pediatric scales daily along with performing quality controls of the glucometers, urinalysis machine and other CLIA waived laboratory tests
4. Checks and documents the temperature on refrigerators for nursing and the laboratory daily.
5. Ensures that assigned exam rooms are stocked with needed supplies.
6. Prepares patients for examination including recording the chief complaint, obtaining vital signs, height and weight, etc. with assistance from the clinic nurse as needed.
7. Obtain vital signs (temperature [oral, rectal, axillary], pulse [radial and apical], respirations, BP, Pulse Oximetry) and reports abnormalities to the clinic nurse or medical provider.
8. Prepares the patient for appropriate exam by the provider. (Reads the previous encounter note from the patient’s last visit.)
9. Collects and assures that ordered laboratory tests are completed. . (May include completing special lab forms). These tests include all CLIA waived tests: Strep A, Urinalysis without micro, blood glucose testing, urine pregnancy testing, HIV quick test, Hemoglobin A1C and Occult blood testing.

11. Obtains immunization reports from the NCIR (North Carolina Immunization Registry) for review by the provider or licensed nurse before patient’s exam.
12. Performs ordered procedures after appropriate training. These tests include EKG, Spriometry Testing, Vision and Hearing Screenings, Cerumen removal and Peak Flow rates.
13. Maintain the confidentiality of Robeson Health Care Corporation and the patients/clients that the Corporation serves.
14. Maintains HIPAA guidelines and Joint Commission readiness.
15. Able to document vital signs, performance of procedures and any interaction with the patient in the Electronic Medical Record.
16. Cleans and arranges the exam room after a patient leaves, in preparation for the next patient, using the exposure control plan guidelines.
17. Functions as a member of the healthcare team by working collaboratively with all members in the medical center. Manages own behavior effectively as a member of the team.
18. Respects the privacy and dignity of individuals and families and provides culturally sensitive care across the lifespan to individuals within a diverse society.
19. Demonstrates ethical and legal behavior in the work environment by maintaining patient confidentiality, respecting the rights of patients. Accepts delegation and assignment within authorized duties of the CNA II.
20. Rotates to other RHCC sites/specialties as needed.
QUALIFICATIONS:
1. Unrestricted listing on the North Carolina Nurse’s Aides Registry as a Nurse Assistant II.
2. Sufficient experience to perform the duties of this position.
3. Must have a valid North Carolina driver’s license.
4. Basic Cardiac Life Support Certificate.
