Social Worker
Job Summary:
As a Social Worker at PACE-RI you'll support and advocate for our participants in achieving their health and social goals. You will be responsible for conducting biopsychosocial assessments to help develop care plans that support participants' health and well-being in collaboration with participants, caregivers, and the interdisciplinary team (IDT).
You'll provide valuable feedback in intake meetings, serve as a liaison between the care team and families, and ensure smooth communication about participants’ services and plans. The Social Worker will also coordinate respite stays and discharge planning, monitor care plans, and attend care conferences to ensure consistency in care across different settings. Addressing behavioral health needs, connecting participants with resources, and helping identify caregiver support needs are all core components of this position.
You’ll also assist participants with benefits access (e.g., housing, Medicaid, SNAP), and help participants with disenrollment and continuity of care. Keeping accurate and thorough records of all interactions in the electronic medical record ensures great participant care. The Social Worker role requires community-based work at partner facilities or in the homes of our participants and their families.
