Physical Therapist Home Health

Home Health Physical Therapist

You would plan and implement specific treatment programs for patients according to the principles and practices of Physical Therapy. 

Your minimum Education, Experience, Licensure, and Certification required is as follows

  • Completion of an accredited Physical Therapy program.
  • PT licensure in the state of Montana
  • BLS Certified and a Valid driver's license. 

Your Essential Functions/Duties- (You must be able to perform with or without Accommodation)

1 Receives physical therapy orders for Home Health/Hospice through the appropriate channels. 

2 Evaluate patients and establish a written plan of treatment with short and long term goals. 

3 Treat patient within established physical therapy guidelines.

4 Keep appropriate documentation records (clinical notes, progress notes, discharge notes, orders etc.)

5 Documents time appropriately.

6 Supervise physical therapy assistants in accordance w/CMS and state requirements.

7 Maintain open lines of communication within department, between departments and with the physicians.

8 Provide appropriate in services to staff members as requested

9 Completes chart reviews and clinical documentation in timely manner 

10. Completes Oasis SOC, recertification’s, resumption of care and discharges per CMS guidelines.

11 Completes other duties as assigned

Knowledge to be able to treat a wide variety of patients in Home Health/Hospice setting. Ability to adapt to fluctuations in work loads. Ability to communicate and work effectively with patients, co-workers, visitors, families, physicians and the general public
