SUMMARY

The Care Coordinator at St. Vincent de Paul Detroit supports the success of its Michigan Energy Assistance Program (MEAP) by providing self-sufficiency services to our neighbors in need of utility assistance.  This role is critical in assisting those seeking utility assistance by providing supportive wrap-around services to protect them from imminent service disconnection, promoting behaviors aimed at conserving energy, and long-term energy self-sufficiency. 

 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: 

 

  • Exemplify the elements of SVdP: Spirituality, Friendship and Service in all phases of work.
  • Advance the mission, goals and objectives in all applicable strategic plans.
  • Support / Live / Adhere to the “Rule” and teachings set forth by Frederic Ozanam.
  • Facilitate workshops aimed at improving financial and energy security.
  • Receive and respond to referrals/appointments for case management services.
  • Meet with program applicants to assess needs specific to their circumstances.
  • Assist with development of applicants’ Energy Security plans.
  • Provide referrals to wrap-around services offered by SVdP, as well as other community agencies.
  • Provide light financial coaching, when appropriate.
  • Provide energy education, when appropriate.
  • Advocate for neighbors, when they are experiencing extreme barriers to connecting with services provided by community agencies and/or utility companies.
  • Follow-up with neighbors, to assure programmatic success.
  • Actively participate in continuous improvement efforts by listening to the “Voice of the Neighbor” (neighbors and Vincentians) and elevating their concerns and suggestions for improvement to the MEAP team during meetings
  • Promote active two-way communication using a variety of methods with Vincentians, neighbors, staff, utility companies, agencies as related to program guidelines, operation and application status.
  • Ensure timely, clear and concise documentation of all services provided.
  • May support SVdPD Conferences by traveling to various service sites to provide assistance with MDHHS MI Bridges applications for utility assistance, Energy Security Plans, Workshops, etc.
  • Other duties, as assigned

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

  • Knowledge and experience in case management best practices, as evidenced by formal education in Social Work, Human Services, Psychology, Sociology, etc., or by at least 3 years’ progressive experience. 
  • Bachelor’s Degree preferred.
  • Computer experience must include Microsoft Office Suite
