Position Summary

The Care Manager is responsible for delivering high-quality in-home care services to aging adults and individuals with developmental disabilities. This role includes assisting with personal care, homemaking, companionship, and community support. Care Managers are expected to provide care with empathy, professionalism, and respect.

Key Responsibilities

  • Assist clients with personal care needs (bathing, dressing, grooming, toileting, mobility support).
  • Provide homemaking services such as light housekeeping, laundry, and meal preparation.
  • Support clients with errands, shopping, and transportation to appointments or community activities.
  • Offer companionship and engage clients in meaningful activities.
  • Monitor client’s condition and promptly report changes to program coordinator.
  • Maintain accurate and timely documentation of services provided.
  • Follow individualized care plans and agency policies.
  • Ensure client safety, dignity, and confidentiality at all times.

Qualifications

  • Must be at least 18 years old.
  • High school diploma or GED preferred.
  • Previous caregiving experience preferred but not required—training provided.
  • Ability to pass a background check and maintain a clean driving record.
  • Compassionate, dependable, and committed to client well-being.
  • Strong communication skills and ability to work independently.

Work Environment & Schedule

  • In-home setting, working one-on-one with clients.
