LICENSED PRACTICAL NURSE (LPN) – Position Summary
The Licensed Practical Nurse(LPN) is accountable for delivering direct patient care under the supervision of a Registered Nurse. Key responsibilities encompass adhering to the established plan of care, administering treatments, and collaborating with team members to achieve favorable patient care outcomes.
LICENSED PRACTICAL NURSE (LPN) – Essential Duties & Responsibilities
Patient Assessment and Care Planning
Provides direct patient care as directed in the plan of care.
Implements current nursing practice following a comprehensive assessment and the plan of care.
Clinical Care and Nursing Interventions
Provides professional nursing care by utilizing all elements of the process.
Provides appropriate preventative and rehabilitative nursing procedures. Evaluates medications and treatments as prescribed by the physician’s plan of care and/or orders.
Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care as directed by the RN or provider.
Provides and maintains a safe environment for the patient.
Utilizes equipment and supplies efficiently.
Patient and Family Education and Support
Provides health care instructions to the patient as appropriate per assessment plan.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
Documentation and Communication
Completes, maintains, and submits accurate and relevant clinical documentation regarding the patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate in the EMR at bedside.
Communicates with the RN or provider regarding the patient’s needs and reports changes in the patient’s condition.
Interdisciplinary Collaboration and Teamwork
Works in concert with the interdisciplinary group.
Attends interdisciplinary group meetings.
LICENSED PRACTICAL NURSE (LPN) – Qualifications/Experience
Graduate of an accredited practical nurse or vocational nursing program.
Two year’s experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
Currently licensed as an LPN in Virginia.
Possesses and maintains current BLS CPR certification.
Demonstrates good verbal and written communication, and organizational skills.
