St. Elizabeth Nursing Home, an Illuminus Community, is seeking a Life Coach (Certified Social Worker) to join the team. In this role, the individual assist with discharge planning, facilitating adjustment to nursing home placement with residents and their families, and maintaining appropriate records accordingly. Currently 

Requirements

 

  • Active Wisconsin Social Work Certification (or ability to obtain within 6 months of hire).
  • Commitment to quality programs and services for the residents/patients/clients.
  • Excellent communications and human relation skills.
  • Ability to relate well to the residents/patients/clients, family members / personal representatives.
  • Ability to maintain and protect the confidentiality of information.
  • Ability to exercise independent judgment and make sound decisions and exhibit flexibility when needed.
