Occupational Therapist

Job Description
FlexPath Therapy Services is seeking a dedicated and compassionate Occupational Therapist (OT) to provide skilled therapy services to patients in the comfort of their homes. This position offers flexibility, independence, and the opportunity to make a meaningful impact in patients’ daily lives by helping them regain independence and improve quality of life.

Responsibilities:

Conduct in-home evaluations and develop individualized treatment plans

Provide occupational therapy sessions tailored to patient needs and functional goals (self-care, home safety, mobility, adaptive strategies)

Educate patients and caregivers on techniques, exercises, and the use of adaptive equipment

Collaborate with interdisciplinary teams (PT, ST, nursing, case managers)

Document all visits accurately and in a timely manner using Kinnser/WellSky EMR

Monitor and report on patient progress and adjust treatment plans as needed

Requirements:

Valid California Occupational Therapy License

Minimum 1 year of clinical experience preferred (new grads welcome)

Home health experience is a plus but not required

Strong communication and time-management skills

Reliable transportation and ability to travel within Los Angeles County and surrounding areas
