Supervisory Social Worker
The purpose of this position is to serve as a Supervisory Social Worker position the duties may include supervisory responsibilities at the VA Boston Healthcare Systems, at the Jamaica Plain location. The incumbent will consult within the HCHV and other departments at the VA Boston Healthcare System.
The incumbent collaborates closely with the HCHV Programs and the Director of the Healthcare for Homeless Veterans Program, performing additional duties as assigned. The incumbent collaborates with the HCHV administrative staff to assist with the necessary documentation and administrative responsibilities required by VAMC, VISN 1 and VACO as well as the various agencies and organizations involved in providing quality care to the program's recipients.
Duties may include, but are not limited to:
- Serves as co-coordinator of the CATCH program and fosters a collaborative and effective team with the existing program supervisor and co-coordinator, senior social worker, and peer support specialists, and responsibilities may include potential supervisory duties.
- Provides professional, clinical and administrative management of the CATCH program in the Social Work Services care line under the umbrella of Healthcare for Homeless Veterans and Social Work Services located at the Brockton, Jamaica Plain and CBOC sites.
- Initiate and continue outreach efforts to inpatient and residential treatment programs, transitional housing sites, Father Bill's shelter in Quincy, Father Bill's Main Spring in Brockton, Pine Street Inn, New England Center fand Home for Veterans, Rosie's Place, St. Francis House and collaborate with homeless outreach teams and community homeless providers across the spectrum on the local, state and federal levels.
- Provides direct community-based clinical and case management throughout the Greater Boston area firstly on an outreach basis and specifically serve veterans experiencing homelessness who live unsheltered and/or in homeless shelters and are demonstrated high users of VA Boston and other emergency room and inpatient services with the mission to assist this vulnerable population with obtaining right-fit housing, resources, and connections to appropriate services and treatment.
- Assists in planning, establishing and implementing policies and procedures; developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating and overseeing the VA Boston Healthcare System's CATCHs Program.
- Support the ongoing replication and evaluation of the CATCH program and trauma-informed care across VA sites nationally in conjunction with the National Center on Homelessness among Veterans, including but not limited to participation in model development, national-level meetings, research and evaluation, education and mentoring, and data collection and analysis.
