Physical Therapist (Per Diem)
Job Summary
Provides Physical Therapy services within scope of practice.
Qualifications
- High School Diploma or equivalent required. Current California Physical Therapy License and Doctorate in Physical Therapy required.
- 1 year of wound care experience preferred. Wound Care certification required within 60 days of hire.
- PT scope of practice.
- Proficient in computer use.
- Able to work under minimal direction and multi-task effectively.
- Communicates effectively with patients/families and all levels of the organization.
- Bilingual (Spanish/English) preferred.
- BLS required within 60 days of hire.
Professional Duties
- Maintain member confidentiality at all times (HIPAA).
- Provide direct planned patient therapy services according to agency policies and plan of care.
- Implements the plan of care and establishes goals and incorporates therapeutic, preventative and rehabilitative therapy services as appropriate.
- Instructs patient and family in care and use of wheelchairs, braces, crutches, canes, prosthetic or orthotic devices, and/or any other assistive device(s) as appropriate.
- Provides health teaching to patient, and/or caregiver regarding treatment and plan of care. Education will utilize terms and teaching methods that are customized to the learning needs of the patient and/or caregiver.
- Coordinates wound services for manager care patients, assists with the establishment of the plan of care in collaboration with the physicians and the UM Case Managers to ensure seamless transition to discharge.
- Provides accurate and timely documentation consistent with the plan of care; This may include preparing clinical and progress notes as appropriate. Documentation will be completed timely and according to organization’s documentation expectations.
- Participates in team meetings and in-service programs as required.
- On-Call and weekend shift coverage as needed
- Collaborates closely with the practitioners and case managers as to the recommended plan of care and regarding any change in condition.
- Consults with the Clinical Director to address and resolve any concerns regarding the delivery of appropriate, timely patient care and education.
Compensation Range
$59.00/hour (Per Diem)
