Supported Living Coordinator/ Care Navigator
Position Highlights: As a Supported Living Care Navigator you will be responsible for providing case management for Supported Living members including oversight of the PCA program and community integration and supervision of assigned staff. Some afternoon and evening hours may be required.

Qualifications:
- Bachelor’s degree and at least two years of relevant experience working in the community with elders, disabled adults or other vulnerable populations.
- Ability to work flexible hours
- Analyzing data, effective listening, facilitating meetings, monitoring activities, planning and problem solving and a working knowledge of computers and basic computer programs.
- Knowledge of age-appropriate activities and behaviors, technical writing, defining issues and drawing conclusions, treatment modalities and assessments.
- Knowledge of community social services, education and health service agencies
- Valid driver’s license, availability of private vehicle, up-to-date car insurance
- Ability to work flexible hours
- Participation in a rotating on-call schedule for the department's emergency on-call cell phone.
- Proficiency with Microsoft Office/Office 365 and word processing skills
- Comfortable with remote application such as Zoom for team or other meetings
