Intensive Case Manager
The ideal candidate will possess the following qualifications:
- Bachelor’s degree in sociology, social work, psychology, or related field; OR high school diploma/GED with relevant coursework, plus two years’ experience in mental health; OR a registered nurse qualification.
- Must have an active driver's license and reliable transportation during working hours.
- One year of experience working with adults in public or private human services is preferred.

Schedule: 40 hours/week; Monday-Friday between 8:00 am-5:00 pm
Earn $19.83/hour or $21.83/hour with the selection of Enhanced Pay
Position Details
The Intensive Case Manager provides high-quality, community-based mental health services to children, adolescents, and adults living in Cumberland and Perry Counties. This role focuses on reducing hospitalizations, promoting independence, strengthening support systems, and supporting vocational and educational goals.
Duties and Responsibilities
- Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
- Deliver Intensive Case Management (ICM) services to youth and adults with serious mental health needs.
- Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
- Support clients in accessing psychiatric care, community resources, and social supports.
- Provide education and support to families and significant others.
- Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
- Participate in clinical supervision, staff meetings, and quality improvement initiatives.
- Participate in on-call rotation for caseload
