Required Documents for submission: • Updated Resume with all gaps explained (3+ years of experience as mentioned in requirement) • Must be a graduated Therapeutic Radiologic Technology or Associate Degree from an institution accredited JRCERT – proof of education is required • Geographic location where key personnel reside – to be specified on the resume. • Active BLS from an American Heart Association (AHA) or Military Training Network program for healthcare providers • Good to have ACLS • Copy of ARRT certificate in Radiation Therapy • 3 Professional references
B.3 PERFORMANCE WORK STATEMENT THERAPEUTIC RADIOLOGY TECH PERFORMANCE WORK STATEMENT (PWS) The contractor shall provide up to three full-time equivalent (3.0 FTE) Therapeutic Radiation Therapists (TRT) and one FTE Certified Medical Dosimetrist (CMD) to the VA Puget Sound Health Care System (Seattle VAMC), located at 1660 S Columbian Way, Seattle, WA 98108, for a total of up to 8,320 hours of service. Contractor employees must perform the required services at the Seattle VAMC for the duration specified in the contract. The contractor shall invoice the VA only for actual hours worked at this location. Overtime and on-call services are not authorized under this procurement. TRT: 3 key people, up to 6,240 hours, with a 1-year base period and four 12-month option years. CMD: 1 key person, up to 2,080 hours, with a 1-year base period and four 12-month option years. General Requirements: Contractor employees shall perform all examinations in the areas assigned to them and meet the standard of care provided by the Seattle VAMC. This shall be in accordance with all terms, conditions, and provisions, and the schedule outlined in this solicitation for a 12-month period from the date of the award, with four 1-year option periods, pending the availability of funds. The base period and option periods shall not exceed 2,080 hours per FTE per performance period.
