Blended Case Manager

Be part of a dynamic team to provide strength-based community case management services to individuals who possess a mental health diagnosis. Duties include; development of recovery-oriented treatment plans, collaboration with assigned treatment team, link with community-based resources and services, provide advocacy and support individual recovery journeys, and coordinate with crisis services through an on-call rotation.

Education: 

• Bachelor’s Degree in Psychology, Sociology, Human Services, Criminal Justice, or related field. 

*$2,500 Sign on Bonus for qualifying position* (Must work full-time – 40 hours per week. The agreement will be reviewed and discussed upon hire.)
