Outreach Coordinator PACE
Position Summary:
Conduct outreach using phone, text and email, to assigned PACE program clients to successfully engage and re-engage them into the PACE program. Carefully review SEAMS & NADAP’s internal database system to document and include all outreach efforts and client interactions in both databases daily.
Essential Functions:
● Generate SEAMS report for all clients assigned to PACE program and provide outreach via various methods to remind clients of mandated engagement appointments and invite them to attend program services either in-person or remotely.
● Responsible for reviewing SEAMS and adhering to internal processes and HRA policies for clients with an ineligible referral.
● Conduct regular and ongoing outreach calls to clients in FTR and FTC status (Failure to Report and Failure to Comply) or who become disengaged within the program. Provide program information and benefits of participation in efforts to increase overall client enrollment within PACE program.
● Review SEAMS and assigned worklist such as the NCO list daily.
● Answer incoming calls, emails and text messages from clients.
● Generate the FTC and FTR list and distribute appropriately.
● Maintain detailed, accurate and timely case notes regarding client outreach into SEAMS NADAP’s internal database system. Case notes must reflect all client interactions and outreach attempts. Update SEAMS demographics.
● Perform other duties as assigned.
Knowledge, Education & Experience:
- Associate degree preferred. High school diploma required with at least one year experience in social services.
- Experience in social services and/or working with public assistance recipients preferred.
- Basic computer skills, including database, email, and online web systems.
- Good customer service skills.
- Bilingual English/Spanish preferred.
