We are NOT a staffing agency. We are Pennsylvania State Licensed, Medicare and Medicaid certified and able to provide a full range of home health and school nursing services as well as IV therapy and wound care.
We are currently seeking a RN Clinical Manager to join our office located in Chadds Ford, Pennsylvania.
In this role, the RN Clinical Manager serves as a supervisor to all field staff to assure adherence to agency policies and federal and state regulations; serves liaison between clinical staff and operations staff; and as liaison among schools, hospitals, the medical community and general community.
DUTIES AND RESPONSIBILITIES:
- Assures all clinical functions meet all state and federal regulations and rules.
- Monitors to assure orders are documented correctly and timely. Provides training and guidance to clinical staff regarding the requirements and agency expectations of orders for disciplines and/or treatments and services. This includes, but is not limited, to communication with home health ordering physicians for start of care orders and timely recertification orders.
- Follows up with intake/staffing coordinators to ensure that referrals results in a timely start of care or are well documented as non-admitted referrals.
- Assures coordination of care between agency care staff. Assures coordination of care between Agency and external providers as required and per agency policy.
- Serves as liaison between CritiCare and physicians, hospitals, schools, families and other relevant agencies in the community regarding patient care and clinical matters
- Keeps the lines of communication open between appropriate personnel to assure the flow of patient care information.
- Provides positive attitude as example of professional behavior for staff both the office staff and direct care staff.
- Provides leadership in orientation and competency verification of agency direct care staff.
- Assures direct care providers meet agency policy requirements.
- Provides direct supervision of new hired clinicians and assures supervision of all staff is completed per regulations and agency policy including documentation of same.
- Participate in the field according to staff needs, providing spot checks and follow-up for specific services and/or quality improvement issues.
- Provides direct response and guidance to staff and patients/clients for problem resolution in patient care.
- Provides information and reports to Administrator as instructed, including but not limited to corrective actions in Plan of Correction or Quality Improvement activities. Reports will be provided timely and without requiring reminders by administration.
- Represents the Agency at all times in a professional and loyal manner. Demonstrates professional deportment and communication style at all times with the following, but not limited to, all office staff, field staff, school personnel, families, patients and physicians and their offices.
- Participates in, by development of and/or presentation of, staff development activities and in-service meetings to educate the personnel.
- Develops and maintains training and in-services on the following, but not limited to, topics required by state licensure, clinical services, rules and regulations, agency policies and expectations.
- Completes, or delegates with administrator prior approval, on-going training as required by agency policy for all supervised agency staff.
- Initiates joint projects and presentations to improve patient care and/or education of community members on issues pertinent to agency interests.
- Participates in the development of identifying future opportunities for home care specialization.
- Serves as a member of the administrative on-call team. Responsible to review on-call logs and assure follow up completed and documented.
- Monitors infection control reports and assure compliance with Infection Control CMS Conditions of Participation.
- Participates, as assigned by Administrator, on compilation of data for the annual agency evaluation and/or quarterly governing body review reports
- Participates in Quality Improvement activities as assigned by Administrator.
- Trains staffs and assures delegation of components of Quality Improvement as needed to meet the total expectations of this position including review of clinical and aide visit notes, orders and documentation.
EXPERIENCE REQUIRED:
- Must have proven supervisory and communication skills.
- Must have proven ability to act as liaison among clinical and non-clinical publics
Licensure, Registration, and/or Certification:
- Current license as a Registered Nurse in the Commonwealth of Pennsylvania. Bachelor’s degree preferred; Associates degree accepted.
- Experience in healthcare of at least five (5) years with a minimum of two (2) years in home health preferred.
- Experience in supervision of at least two (2) years.
- Cardio Pulmonary Resuscitation (CPR) certified
- Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with Pennsylvania laws.
- IndentoGO FBI Fingerprints clearance
- PA Child Abuse clearance
- 2 Step PPD test results or chest x-ray for TB screening
This is an exempt full-time salary position based in our office in Chadds Ford.
