Care Coordinator - Public Health & Transitions

🔹 Job Description:

Are you energized by fast-moving environments and meaningful, hands-on work? Circle of Care St. Louis is seeking a Care Coordinator II – Public Health & Transitions, a hybrid role designed for individuals who excel in dynamic settings and want to make a direct impact on the lives of older adults and individuals with disabilities.

This full-time position combines in-home personal care, care coordination, and community outreach — and is ideal for someone who thrives on variety, adaptability, and problem-solving. No two days are the same.

 

In This Role, You Will:

Provide direct in-home care to support clients with daily living (20+ hrs/week)

Help clients navigate benefits like Medicaid, SSI, and subsidized housing

Visit hospitals and skilled nursing facilities (SNFs) to coordinate discharge plans

Respond to rapidly changing client needs and shifting care priorities

Represent Circle of Care at health fairs and community wellness events

Document client progress and care coordination using Caresmartz360 and Charity Tracker

🎓 Minimum Qualifications:

Associate degree in human services, healthcare, or public health
OR

CNA certification with 2+ years of direct caregiving experience

Reliable personal vehicle, valid driver’s license, and auto insurance

Comfort using smartphones, tablets, and digital systems

Must pass Missouri Family Care Safety Registry background screening

 

Preferred:

Bachelor’s degree in Social Work, Public Health, or Human Services

Experience with Medicaid-funded programs or SNF discharges

Bilingual skills (Spanish, Bosnian, Vietnamese, etc.)
