Who We Are:
This is a unique and exciting opportunity to join and contribute to the Dual Treatment Track program. The DDT program is a jail diversion initiative designed for individuals who meet the criteria for a diagnosis of Serious Mental Illness (SMI) and/or co-occurring disorders within the criminal justice system. This collaborative effort involves Chesterfield Mental Health Support Services and Chesterfield/Colonial Heights Community Corrections Services, which together offer both clinical and probation services in a community setting. The program is looking for a qualified candidate who is passionate about serving the SMI population, enjoys a varied work environment, and can adapt to different settings, including an office, a jail, and the community, in order to provide comprehensive care for this vulnerable group.
Minimum Education and Experience:
Master’s degree in human services or related field; one year of experience related to assigned program; or equivalent combination of training and experience. Professional licensure, such as Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or license-eligible is preferred; experience in community behavioral health services preferred; experience in a criminal justice setting or with justice-involved populations preferred.
Required Knowledge, Skills, and Abilities:
Working knowledge and understanding of fundamental concepts of normal and abnormal human behavior, personality development, family and group dynamics; knowledge of and an ability to apply a variety of psychotherapeutic supports and/or treatment techniques; adherence to basic ethics appropriate for a professional helping relationship; knowledge of mental health diagnoses, assessment, and/or psychological and educational testing; some knowledge of consultation program; knowledge of psychosocial history development; very good interviewing and interpersonal skills; ability to communicate clearly, both orally and in writing; demonstrated ability to act with good judgment; computer proficiency skills; working knowledge of electronic health record systems. Knowledge of documentation and billing practices for targeted case management preferred.
Additional Requirements
Current valid driver’s license and good driving record required. Based on the Virginia DMV point system, records must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.
Pre-employment drug testing, FBI criminal background check and education/degree verification required.
Duties include but are not limited to the following:
• Evaluates individuals' strengths, needs, and resources to create person-centered individualized service plans that address their identified needs and goals for services;
• Assists individuals by connecting them to necessary supports, resources, and services, monitors their participation in these services and track progress towards their goals;
• Provides supportive counseling and facilitates psychoeducational groups on mental health and substance use;
• Completes documentation in a timely manner in the electronic health record, following licensure and Department of Medical Assistance Services (DMAS) standards;
• Conducts thorough behavioral health assessments, evaluations, individual service plans, quarterly and annual reviews, progress notes, and service authorizations;
• Collaborates within a multidisciplinary team setting to ensure excellence in customer care; and
• Performs other work as required.
