Position: Licensed Social Worker (Hospice Care)
Location: Lake County, IL
Compensation: $57,000-$69,000 & Benefits

Overview:
We are seeking a compassionate, licensed Medical Social Worker to join our interdisciplinary hospice team. This role focuses on assessing and supporting the psychosocial needs of patients and their families, ensuring quality end-of-life care in accordance with hospice philosophy and organizational standards.
Key Responsibilities:
- Assess patients’ psychosocial status and coordinate with clinical team
- Provide counseling and support to patients and caregivers
- Develop individualized care plans and assist in care coordination
- Maintain accurate clinical documentation
- Serve as a liaison to community agencies and resources
- Attend interdisciplinary team meetings and contribute to quality improvement
- Educate staff and families on social and emotional aspects of terminal illness
Qualifications:
- Bachelor’s degree in Social Work (CSWE-accredited); licensure in Illinois required
- At least 1 year of healthcare experience; hospice experience preferred
- Strong communication, organizational, and documentation skills
- Valid driver's license, insured vehicle, and current CPR certification
