Social Worker - LICSW - AYA Limited Tenure
Description
Collaborates with multidisciplinary teams, provides psychotherapy, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the section of Social Work and the Department of Nursing.
- Acts on needs assessment for AYA patients completed by the AYA navigator
- Contributes to ideas for innovative ways to engage AYA patients.
- Provides a Psychosocial Assessment and identifies interventions such as crisis intervention, individual/couples/family counseling, referrals to psychiatry, and connection to resources.
- Addresses the needs of patients with complex psychosocial risks.
- Formulates, coordinates, and implements the psychosocial component of patient care.
- Collaborates with the healthcare team and recommends interventions to address wholistic patient care.
- Initiates and facilitates interdisciplinary meetings.
- Continually assesses and addresses quality of life and end of life concerns and issues.
- Assesses patient and family level of understanding, barriers to learning, and provides age-specific education and support.
- Assists in the development of age-appropriate interventions, tracking the use of interventions, and sharing with the broader AYA community.
- Helps develop and implement surveys, needs assessments, and evaluations to aid in program development and evaluation.
- Compiles relevant and up-to-date information and resources pertaining to oncology and AYA care. Shares resources with program manager, RN navigator, and patient navigator.
- Facilitates AYA support groups. Co-facilitates the AYA Patient Advisory Council with the program manager.
- Collaborates with community and governmental agencies and resources as needed.
- May Provides supervision and mentorship to graduate students and volunteers.
- Participate in patient research as assigned.
- Collaborates in publication ideas, submissions, and presentations.
- Assists in the planning of conferences and educational programs.
