Home Health - Full Time Physical TherapistLocation: North Dallas, TX
Sign-on Bonus: $15000
Location: North Dallas , Near Downtown, Far North Dallas, Garland
Job Summary
The Home Health - Physical Therapist will be responsible for providing physical therapy services to patients in their homes. The Physical Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed. The Physical Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.
Responsibilities
- Conducts patient assessments and assists the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests
- Assist in the development of the Plan of Care
- Directs and aids patients in active and passive exercise, muscle re-education; as well as gait, functional, ADL, transfer, and prosthetic training
- Treats patients to relieve pain; develop and restore function; and maintain maximum functioning using means such as exercise, massage, heat, water, light and electricity
- Makes use of ultraviolet and infrared lamps, diathermy, ultrasound, whirlpool, contract baths, and applies moist packs
- Observes, records, and reports to the physician and the supervising therapist the patient’s response to treatment and changes in the patient’s conditions.
- Instructs patients, significant others, and staff in the care and use of wheelchairs, prosthetic, and orthopedic devices; and in areas of physical therapy in which they can
participate and assist the patient. - Visits patients according to the Plan of Treatment; completes a progress note for each visit; and submits progress notes timely.
- Attends all education and in-services as required.
- Prepares a written discharge summary and a written summary report on a patient's condition at least every 60 days.
Requirements
- The candidate must be open to working in the Fort Worth and Weatherford areas.
- Current state license as a Physical Therapist
- Graduate of an American Physical Therapy Association approved physical therapy program.
- Minimum of 1 year of experience in home health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent time management and organizational skills
- Valid driver's license and reliable transportation
