As Michigan’s largest nonprofit provider of senior services, Holland Home of Grand Rapids offers a full continuum of care. Holland Home has beautiful residential campuses and a hospice residence to offer in-patient end of life care. Our community services are provided through Faith Hospice, *ATRIO Home Care, *ATRIO Help at Home, *Care Resources and *Reliance Community Care Partners. Employing almost 1,500 and attending to more than 4,100 residents and patients daily, Holland Home follows a strong Christian mission of serving with love and compassion while demonstrating a commitment to excellence for over 126 years in the greater Grand Rapids community.
*ATRIO is a faith based partnership consisting of three agencies Holland Home of Grand Rapids, Clark Retirement and Resthaven in Holland, MI. ATRIO provides services in Ottawa, Allegan, Barry, Ionia, Montcalm and Kent counties.
*Care Resources is a Program of All-inclusive Care for the Elderly (PACE®). Care Resources was developed as a health care program for older adults needing long-term care and wishing to age in place. As we age, medical problems become more complex and last longer. Care Resources is responsible for all primary and specialty care for those who are enrolled.
*Reliance Community Care Partners is a non-profit health care and case management organization that specializes in coordinating community health care services for older adults and those with disabilities.
